Nurse's Corner

Dear CDS Families with children enrolled for the 2018-19 school year,

For the upcoming school year, we will be shifting all student medical information to an online database.
 
There are many benefits to this change including less paperwork for parents to complete each year and having more accurate information that is accessible to parents and the appropriate employees at our school. The healthcare management system we have adopted is called Magnus Health SMR (Student Medical Record). As this is a web-based system, you will have continuous access to your child’s health record as well as the ability to make updates when needed. You will also have the option to take the digital health file with you after your child graduates.

On Tuesday, May 29, you will receive a Magnus Health welcome email with your login information. Please use the link in the email to verify your account and reset your password. You will also have access to your Magnus Health SMR account through the Chatham Day School website. You will access the account via the button titled “Magnus Health Portal” which is found in the "Nurse's Corner" under "School Life" on the website. We ask that you enter the health information required within your Magnus Health SMR account for each child attending Chatham Day School no later than August 15, 2018.
 
Timely submission of health information and other forms is vital so that preparations can be made prior to the first day of school. You will receive weekly reminders as long as there are outstanding items in your account. Please reserve 20 to 30 minutes per child, depending on particular circumstances. The good news is that you will not have to repeat this work in subsequent years: the data is securely stored, so going forward you will simply provide updates to the record as needed.

To access your Magnus Health SMR Account on Tuesday, May 29 and thereafter, please use the button titled “Magnus Health SMR Portal” above.

Below is a checklist of items that are to be entered in Magnus. You will find additional details and explanations within the system, as you proceed through the requirements.
Mandatory for all students:
  • Student Vital Health Record
  • Immunization Documentation - a copy of the pediatrician’s immunization record  
  • Annual Health Physical Information (Preparticipation Physical Evaluation)
  • Consent to Treat
  • Consent to Health Screening
If applicable, for individual students:
  • Medication Form
  • Asthma Action Plan Form
  • Food Allergy Action Plan Form
  • Diabetes Action Plan Form
  • Seizure Action Plan Form
  • State of NJ Health History Update Questionnaire
  • Sudden Cardiac Death in Young Athletes Pamphlet and Sign-Off Sheet
  • Concussion Parent and Student Form
